Phoenix Service Corporation provides an array of services to promote independent living. We educate, advocate, and provide supportive services to empower individuals with physical and mental health challenges. Phoenix Service Corporation strives to advance the independence, productivity, and confidence of people in the community.

24-hour Emergency Assistance

Provides on-call support, resource sharing, and problem solving. This service may also include immediate response for assistance at a person’s home due to a health or personal emergency.

Homemaker Services

Homemaker Services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living. Services are customized to meet the needs of the individual.

Individualized Home Supports without Training

Service to help individuals who have difficulty accessing the community. Focuses on decreasing isolation and developing supportive relationships and community connections through 1:1 support. This service can be provided in their home or in the community.

Individualized Home Supports with Training

Individualized Home Supports help an individual to develop, maintain, and improve community living skills. They assist individuals by developing personal goals and provides 1:1 support to help with a variety of areas. The goals are geared towards successful, independent living; a support plan may include improving their health and housing, completing important paperwork, developing supportive relationships, obtaining community resources, or finding employment or volunteer work.

In-Home Family Support

Training services are provided in a person’s home and in the community, with the goal to maintain or increase the person’s physical, mental, emotional, and social functioning. Training is available to the family, including extended family, of the person with a developmental disability.

Housing Support Program

The Housing Support program is a state-funded program that provides an income supplement to approximately 20,000 recipients each month to pay for rent and food. For more information, visit our Housing Support page.

Residential Habilitation (SLS)

Services provided to a person who cannot live in their home or who need outside support to remain in their home. Habilitation services are provided in the person’s residence and in the community, and are directed toward increasing and maintaining the person’s physical, intellectual, emotional, and social functioning.
