Independent Living Skills Specialist

Job Description:

Phoenix Service Corporation (PSC) has an exciting opportunity for someone who would like to make a difference in the community! PSC is now accepting applications for a part-time Independent Living Skills (ILS) Specialist. Please note this position is part-time with up to 60 hours, including no weekends, evenings or holidays.

The ILS Specialist provides training and assistance to individuals who need a little extra assistance by promoting independent living and decreasing dependence on caregivers. For example, ILS Specialists teach individuals how to budget and manage money, access community resources, coordinate medical care, or maintain their home.

The ILS Specialist is the liaison between clients and outside agencies, including vocational, rehabilitative, cultural, recreational and medical services. The ILS Specialist advocates for the individuals and their needs, conduct assessments, coordinate ILS program plans, and meet weekly with individuals. The ILS Specialist is also responsible for completing and maintaining the necessary documentation for ILS services.

Job Requirements:

  • Previous related field experience preferred, such as assisting people with disabilities and/or in the Health and Human Services field
  • Experience with person-centered planning approaches
  • Knowledge of disabilities and/or chronic illnesses
  • Knowledge of independent living skills management and ability to coordinate activities and services
  • Must be able to work independently without direction or supervision
  • Ability to read, write and speak the English language
  • Ability to travel among agencies, hold a valid driver’s license, and maintain a safe driving record
  • Excellent communication skills, both written and verbal,  and possess exceptional listening skills
  • Ability to work with diverse clients

Adult Companion Support Professional

Job Description:

Phoenix Service Corporation (PSC) has an exciting opportunity for someone who would like to make a difference working with adults with physical and mental disabilities! PSC is now accepting applications for an Adult Companion Support Professional. Please note this position is part-time.

The Adult Companion Support Professional provides companionship, assistance and/or supervision of the PSC client in their home or community. Activities may include light housekeeping tasks, meal preparation, shopping and errands, helping with laundry, and companionship. This position requires flexible work hours and may require travel throughout the Twin Cities metro area. The Adult Companion Support Professional acts as an advocate for individuals and their needs.

Job Requirements:

  • High School Diploma or equivalent
  • One year of experience with people with disabilities and/or in health and human services preferred
  • Knowledge of traumatic brain injuries, mental health diagnoses, and related disabilities
  • Communicates effectively with clients, families, and outside agencies, while displaying positive, supportive, professional conduct and presenting oneself as a role model
  • Excellent communication skills, both written and verbal
  • Ability to coordinate activities and services
  • Ability to read, write, and speak the English language
  • Ability to travel among agencies, hold a valid driver’s license, and maintain a safe driving record
  • Ability to complete and maintain all documentation required for Adult Companion Services
  • Adheres to all rules, regulations, policies and philosophy of Phoenix Service Corporation and its licensing agencies
  • Ability to lift up to 75 pounds, including lifting objects and transferring people. Must be able to stand, stoop, bend, reach, and push/pull wheelchairs frequently.
