Waiver Case Manager

Job Description:

Phoenix Service Corporation is now accepting applications for a Waiver Case Manager to join our team of experienced professionals who specialize in meeting the needs of adults with disabilities.

The Case Manager is responsible for coordination of services for vulnerable individuals and assisting with navigating complex service delivery systems.  The Case Manager assists persons in gaining access to needed social, medical, educational, and other supports and services. They work on behalf of the person along with the interdisciplinary team to identify their unique needs and to minimize the impact of the disability on the person’s life while assuring continuity of services and supports. They act as an interdisciplinary team member and advocates for their client.

Our goal is to provide the services most appropriate to the needs and desires of the individual. We focus on what is important to the person.

Job Requirements:

  • Public Health Nurse, Registered Nurse, or graduate of an accredited four-year college with a BS/BA in social work, psychology, sociology, or a closely related field OR a graduate of an accredited four-year college with a major in any field and one year experience as a social worker in a public or private social service agency
  • Knowledge of person-centered principles and plans
  • Adheres to and maintains knowledge of various rules, regulations, and state and county policies regarding CADI, DD, and BI waiver programs.
  • Communicates effectively with clients, families, coworkers, and outside agencies.  Displays positive, supportive, cooperative and professional conduct when doing so.
  • Excellent communication and interpersonal skills, problem-solving, and organizational skills
  • Ability to work with clients with a variety of cultural, medical, psychiatric, and physical needs
  • Telephone and computer skills. Will participate in frequent phone calls, desk work, and computer work
  • Ability to read, write and speak the English language
  • Ability to see and hear adequately to respond to emergency situations.
  • Ability to travel throughout the Twins Cities metro area, holds a valid driver’s license, and maintains a safe driving record
  • Ability to frequently walk, stand and sit
